What we collect: We may collect your name, address, date of birth, contact details, medical history, treatment records and comunications methods. Payment details are only collected on our secure payment device not at our practice.
How we use your information: We use your information to ensure that you have a safe and effective experience in terms of your dental care, for managing appointments, processing payments and communicating to you about your dental care and our practice.
Sharing of information: We do not share information without your consent. We may have to share relevant information to laboratories, NHS referral centres, our sister dental practices or regulatory bodies if legally required. We never sell your data or share with anyone not needed for your dental care.
How we store your data:
Your information is now securely stored electronically in line with GDPR.
Your rights: You are entitled to view your dental records or request that we delete your personal details.
You have the right to access the data that we hold about you and to receive a copy. Access may be obtained by making a request in writing outlining what you require. We only keep up to 11 years of paper records.
We will levy a fee of £10 for records that are copied. If a request is made to copy x-rays, an extra charge of £40 will be made. We will email a copy of the record within 30 days of receipt of the request and fee and an explanation of your record should you require it.

Pricing:
We are transparent with our charges. We do not offer NHS services and charge accordingly to our expertise.
Our price list is a guide on our website and dependent on many factors, which will be discussed with you by your dentist.
Individual treatment plans accord for individual needs and will be discussed by your dentist. Full consent is needed before any treatment commences.
Payment terms:
Payment is due at time of treatment or before. We may ask for deposits when booking your appointments.
Missed appointments:
We understand emergencies may arise, but please let us know as soon as this happens. Ideally, we need 48 hours cancellation notice as we are a busy practice.
If this is not given then we may have to charge you for the dentist's time.
Repeat missed appointments will lead to the patient being removed from our registers.
Safeguarding:
We are commited to safeguarding children and vulnerable adults in our practice as patients. Therefore, we ensure that : All out staff have had safeguarding training and follow local safeguarding guidelines. If we feel that there is a safeguarding issue then we will esclate this with safeguarding authorities in line with our duty of care and the law.
If you have any safeguarding concerns, then please contact our safeguarding lead at the practice.
Accessiblity:
Our practice is wheelchair friendly, we will need to know when your book your appointment so facilities are available to you.
We can not do dominicary visits to patients.
Regulations:
Our practice is regulated by the Care Quality Commissioner (CQC) and inspected at certain intervals.
